INTENDED COMPETENCY
To enable those involved in projects and plant or installation modifications to contribute meaningfully in Hazard and Operability Studies. This will ensure that the design and operation meet all safety, health and environmental as well as operability requirements. Further appropriate post-course experience should enable mastering the skills to take on a Hazard and Operability Study Leader role.

OBJECTIVES AND ACHIEVEMENTS

§ Understand plant and equipment failure modes and consequence effects
§ Comprehend piping and instrumentation diagrams in relation to control and protection instrumentation configuration
§ Grasp the principles of Hazop study and the development of guide words for analysing designs
§ Explain the scope, procedures and limitations that are applicable to a HAZOP study
§ Define the "design intent" of a study section and break a process down into defined sections
§ Identify generic causes of deviations and their potential consequences
§ Assess acceptability of existing prevention and protection safeguard measures
§ Generate and evaluate possible risk control options that will sustain the design intent and meet SHE standards
§ Acquiring Hazop study procedures and templates for use back at work
§ Organising and leading of Hazop studies, manage and handling of team problems
§ Creation of awareness of other Hazop study stages in a complete project cycle

DAY 1
 Legal aspects (OHS, MHS, HCS, Environmental & Water Acts, etc.)
 Summary of equipment failure (mechanical, instruments) and human factors.
 Review of plant and equipment hazards (fire, explosion and toxic release).
 Overview of six-stage study system.
 Aspects of the Preliminary Hazard Review (HAZOP 1).
 Team exercise (HAZOP 1).
 Video: Major disaster.

DAY 2
 Plant and Equipment Hazard Assessment, Process Hazard Analysis (PHA) (HAZOP 2).
 Team exercise (HAZOP 2).
 Principles of detailed examination HAZOP 3.
 Team exercise: developing deviations from standard guidewords and applying to a simple operation.
 Applying the HAZOP 3 technique to a continuous process.
 Team exercise: continuous process.
 Applying the HAZOP 3 technique to a batch process.
 Team exercise: Drawing up a batch sequence table.

DAY 3
 Team exercise: Batch process
 Applying Hazop 3 technique to a procedure
 Applying the HAZOP 3 technique to a mechanical system.
 Applying the HAZOP 3 technique to an electrical system.
 Overview of Hazards of construction study (Hazcon).
 Failure mode effect analysis.
 Overview of HAZOP 4.
 Overview of HAZOP 5.
 Overview of HAZOP 6.
 Layer of Protection Analysis (LOPA).
 Team exercise of using Safety Integrity Level (SIL) classification in the Hazop study.
 Overview of electrical area classification and an exercise to illustrate the application.
 Final examination
